This practical toolkit shows how Humber and North Yorkshire Integrated Care Board developed the People Story dashboard which is an open-access tool that brings fragmented data together into a single, trusted source. Used by over 40 organisations, it shows how clear, accessible data can support neighbourhood-level insight, joint planning and more sustainable health and care decisions.
